Programs/Services

Nurse Practitioners provide comprehensive, accessible, and coordinated family health care services through Nurse Practitioner-Led Clinics to populations who have difficulty accessing a primary care provider (i.e. unattached patients). Nurse Practitioner-Led Clinics deliver :

  • Comprehensive primary care to patients in the community ensuring continuity of care;
  • Holistic care that addresses all needs of each patient;
  • Enhanced health promotion;
  • Chronic Disease and illness management and prevention;
  • Care coordination and navigation of the health care system;
  • Walk-In Clinic for LNPLC patients – Mondays and Fridays from 1pm to 4pm; and
  • Integrated care within the community through established partnerships.

Pap-a-palooza

During the month of April, 2017, we are reserving appointment times for PAP tests for cervical cancer screening for anyone in Thunder Bay who requires one.

Partnering with the Thunder Bay Regional Health Sciences Centre, this program is offered to keep everyone up to date on cervical cancer screenings.

Caregiver Support Program

Are you the caregiver for a patient of our clinic, or a family member?

This comprehensive program is open to the public, and will be running again this spring Tuesday afternoons, May 2 – June 6, 2017.  It will be hosted by a number of our clinic’s providers including Social Worker, Dietitian, NP, and local organizations.

Chair Exercise & Falls Prevention Program

We are partnering with VON to offer their exercise program at our clinic.  Starting May 4, 2017, and being held every Thursday at 2:00, this program is open to the public, anyone 55 years or older, and is drop-in style, so no appointment is needed.

Who can attend?

This program is specifically designed to help people who would like to:

  • Improve their balance
  • Improve their quality of life
  • Decrease their chances of falling
  • Participate in a social setting
  • Learn more about preventing falls

Memory Screening Clinic

How is your memory?  Come in for a free screening!  June 13, 2017 and see an NP, Social Worker, Pharmacist and Registered Dietitian

We hope to see: anyone 75 years or older, or anyone 65-74 with: diabetes, heart disease, a previous heart attack or stroke, high blood pressure, a history of smoking, high cholesterol, atrial fibrillation, obesity, a family history of heart attack or stroke before the age of 60

The next sessions will be held September 19 and November 28, 2017

Skin Screening

If you need something checked on your skin, we have a half-day clinic running on June 15, 2017 where you can have a skin exam from our NP and on-site treatments for minor concerns from our RPN.  You can also be referred for a biopsy or to a specialist if needed.
