      Hello Doug

Please have your wife complete a patient registration form, which is found by clicking the link on the homepage of our website. It can be dropped off at the clinic, faxed to us at 475-9797, or scanned and emailed to lnplcregistration@tbaytel.net. We are currently going through our wait list to accept new patients and could process the application within a few months. However, until she is accepted as a patient here, she should continue to see her current primary care provider. If you have any questions, please call the clinic at 475-9595 and speak to reception regarding the patient application process. You can also speak to your NP or health care provider next time you are at the clinic. All the best!
